For patients scheduling who are under 18 years of age (19 in Alabama) please make sure you have permission from your parent or legal guardian to schedule this appointment. Your parent or legal guardian must accompany you on your initial visit and on certain subsequent visits to provide appropriate informed consent.
Please note that at this time, we do not accept Gateway Medicaid. We do accept Gateway Medicare.
If you have an HMO or VA Community healthcare plan that requires a referral or verification, please call 724-471-7100 to schedule your appointment.

- Cosmetic visit or complete the cosmetic appointment request form
- Surgical visit, including MOHS surgery
- Light treatment
- Laser treatment
- Injections
- Patch testing
- Wound check
- Stitch/suture removal
