For patients scheduling who are under 18 years of age (19 in Alabama) please make sure you have permission from your parent or legal guardian to schedule this appointment. Your parent or legal guardian must accompany you on your initial visit and on certain subsequent visits to provide appropriate informed consent.
Please be aware that we do not accept Medicaid at this time. If you have an HMO plan, you must have your referral ready at the time of your visit.

To schedule a biopsy or excision, please call our office at 708-788-3885, Option 3.
If you need to schedule an appointment with a Physician not currently included in our online scheduling tool, please call 708-788-3885, Option 1.
